Online therapy costs without insurance

Online care · Practical guide

Compare per-session fees, full subscription payments and the services included in an online therapy quote.

The practical answer: Compare what you can actually book, the length and number of sessions, and the full billing period. A weekly headline, a monthly subscription and a per-session fee are different price units.

Three different self-pay models

The following are published US examples checked on November 3, 2026. We selected contrasting payment models, including providers outside our affiliate relationships. They are not ranked by treatment quality. Confirm the available clinician, schedule and final quote before enrolling.

ServicePublished price modelWhat the fee covers or leaves open
Brightside Therapy 1$299 per monthFour video sessions and messaging. The FAQ lists additional 55-minute sessions at $59 each. Psychiatry, medication and combined plans require a separate comparison.
Talkspace 2From $69/week messaging; $99/week video + messaging; $109/week with workshopsIts help page describes recurring monthly, quarterly or biannual billing. Live plans offer up to four 30-minute sessions per billing month, subject to availability. Obtain the actual period charge.
Open Path 3$65 one-time membership, then $40–$70 per individual sessionFor people meeting its financial-need criteria. Session fees are paid to the therapist. Find a suitable available online therapist and agree the fee before joining.

Translate the price into a useful budget

Start with the amount your payment method will be charged, rather than a price divided into smaller units. Then list the sessions you intend to use. A subscription is not automatically a per-session service, and unused access may not be refundable.

At Brightside’s stated $299 rate, dividing by four included sessions gives a $74.75 per-session allocation. That is arithmetic across the package, not an option to buy a single session for $74.75. If you attend fewer sessions, the monthly charge is still the relevant budget figure.

For an eligible Open Path member attending four individual sessions at the published $40–$70 range, the first four sessions plus the one-time $65 fee would total $225–$345. Four later sessions at the same agreed fee would be $160–$280. This illustration assumes four sessions are available and booked; it is not a monthly package or an eligibility promise.

Multiplying a weekly headline by four produces a four-week illustration, not proof of a calendar-month invoice. Talkspace’s stated billing choices make the checkout total especially important. Ask how long the period lasts, how many live-session credits it provides and when credits expire; its help page says unused live credits do not roll over.

Questions to answer before a subscription starts

  1. How many live sessions are included, how long is each session, and are messaging and workshops separate services?
  2. Can the clinician offer appointments in your state and at times you can attend? Paying for access is not the same as securing every desired appointment.
  3. What happens to a late cancellation, missed appointment, unused credit or period when the clinician is unavailable?
  4. What is the exact next charge and how do you stop it? Request written confirmation and keep the billing-period end date.
  5. Are medication, psychiatric evaluation, additional sessions, reports or other services billed separately?

If you are not using insurance, ask the treating provider about a written good faith estimate where applicable. CMS explains when estimates are normally required and how they differ from a bill4. This is a route to clearer expected charges, not a guarantee that every unexpected cost will be refunded.

Common payment questions

Is messaging-only therapy the same purchase as weekly video sessions?

No. Compare the stated service and contact format before comparing prices. A lower messaging-plan headline does not establish that live appointments are included.

Does cancel anytime mean a refund of the current period?

Not necessarily. Stopping future renewal and refunding already paid access are separate questions. Check the specific plan’s billing and refund rules before paying.

Can I assume a quoted insurance copay applies to me?

No. This guide concerns self-pay examples. If you have coverage, verify the actual provider, network and personal cost with your insurer and the service. Do not replace your own quote with a platform’s average copay.
